If you would like to request funding to support children or youth in Chilliwack, please contact us to discuss your application. Please note that the Optimist Club Board does not meet in July and August. Any requests received during these months will be addressed at our September Board meeting.

Please note: There are 2 separate forms, please select the appropriate form:
- Fund Request Form – ORGANIZATION (example: for clubs, teams, organizations etc.)
- Fund Request Form – INDIVIDUAL (example: for the needs of a family or child)
Request forms can be filled out on your computer, emailed to optimistclubofchwk@gmail.com (preferred method). Please ensure all required fields are completed before submitting.
Completed forms must be submitted to the club by the first Thursday of the month to be considered at the next board meeting.
